1. Original Death Certificate and BI-1663 Notification of Death Form

2. Identity Document of deceased

3. Name, address of employer and salary number of deceased

4. Details of Pension Fund/s

5. Name, address and reference number of Medical Aid Scheme

6. Income tax details (including details of accountants)

a) Name, address and telephone number of deceased's house doctor

b) Name, telephone and case numbers from Police Station should the

deceased have died of unnatural causes (e.g. motor vehicle accident,

suicide etc)

7. Particulars of Business (if deceased was owner): Close Corporation, Company, Partnership and key agreements

a) Names and addresses of shareholders, members or partners

b) Name and number of Auditor / Accounting Officer

8. Details of divorced and predeceased spouses

9. Ante nuptial contracts, divorce agreements and maintenance agreements

a) Title deed in respect of fixed property

b) Rates and taxes account and / or details of Body Corporate or the Managing Agents

10. Original registration certificates in respect of motor vehicles

11. Firearms (to be handed to the Executor for safekeeping )

12. Certificates / details in respect of timeshare

13. Share Certificates (stock exchange or private companies)

14. Hiring and letting contracts

15. Cheque books, investment statements, credit cards, ATM cards

16. Insurance Policies - whether payable to the estate or to a third party

17. Details of short-term insurance (motor vehicle, contents of house etc)

18. Accounts owing by the deceased

19. Copies of Identity Documents of beneficiaries and marriage details

20. Copies of birth certificates of minor beneficiaries

21. Estate duty paid by the estate of the first dying spouse
